Form 100: the Georgian medical certificate most often translated

Form IV-100/a, known to everyone in Georgia simply as Form 100, is the standard extract from a patient's medical record issued by hospitals and clinics. It contains the patient's identity details, the period of treatment, the diagnosis with ICD-10 codes, the treatment carried out, examination results, recommendations and the signatures and stamps of the treating doctor and the institution. It is the document a foreign clinic asks for when a patient continues treatment abroad, the document an insurer requests for a claim, and the document a court or employer receives as proof of illness or fitness.

Translating Form 100 well requires two different skills at once. The header, the institutional details, the stamps and the signature block are administrative and must be reproduced in the layout the reader expects. The clinical body, which is often written in dense medical shorthand with Latin abbreviations and Georgian or Russian clinical terms, must be rendered in the terminology a doctor in the destination country uses. We translate diagnoses using the ICD-10 wording of the target language, keep the codes, expand abbreviations where the meaning is unambiguous and preserve them with a note where it is not, and reproduce laboratory values with their units unchanged.

Form 100 is frequently issued in handwriting. We ask for the clearest possible scan, mark genuinely illegible words rather than guessing, and, when the client is still in contact with the clinic, suggest requesting a typed version, which many hospitals now provide from their electronic records.

Health certificates for visas, work, study and licences

Beyond treatment, medical certificates are required by a long list of administrative procedures. Work visas and residence permits for the Gulf states, several Asian countries and some EU countries require a health certificate confirming the absence of specified infectious diseases, sometimes on a prescribed form. Universities abroad ask for a health certificate and vaccination record before enrollment, and student dormitories often require a chest X-ray report. Employers in healthcare, food handling, transport and education require fitness-for-work certificates. Driving licence exchange in many countries requires a medical certificate of fitness to drive. Adoption, surrogacy and IVF procedures involve extensive medical files. Athletes need fitness certificates for competitions and licences.

Georgian clinics issue these certificates in Georgian, occasionally with an English version for certain embassies. When the destination requires a notarized translation, we prepare it; when the embassy accepts a certified translation on our letterhead, we say so, because notarization adds cost and a day and is unnecessary for many visa centers. Vaccination records for children entering schools abroad are translated with vaccine names given in the international nomenclature and dates preserved, since school nurses compare them against the local schedule. Sick leave certificates are translated for foreign employers and for social security claims, with the period of incapacity and the issuing doctor clearly stated. For treatment-related documents, we recommend a certified translation rather than a notarized one unless an authority is involved, and we recommend sending the full record instead of a selection: a doctor reading a translated epicrisis without the laboratory results it refers to will ask for them anyway. Medical translation errors have direct consequences, so every certificate is translated by a translator who works with medical texts and checked against the source for numbers, units, drug names and dosages, dates and laterality (left and right are among the most common errors in careless medical translation). Where the source is ambiguous, we ask rather than assume, and where a diagnosis is written in a form that has no exact equivalent in the target language, we give the closest recognized term and preserve the original in brackets.

Medical documents contain the most sensitive personal data a client will ever send us. They are handled by the translator assigned to the job and, where notarization is involved, by the notary, and are not shared or used for any other purpose. Send the clearest possible scan of every page, including stamps and the reverse side if it carries text.
