What types of medical documents do we translate?

Medical translation covers many different documents. Sometimes a client needs a single page translated, for example a medical certificate or a diagnosis. In other cases a full medical history, Form 100, a discharge summary, a set of test results, clinical trial documentation or a pharmaceutical leaflet is translated. Each document has its own terminology and format, so it is always better to give an assessment after seeing the document.

Translation of Form 100

Translation of Form 100 is one of the most requested medical translation services. Form 100 often contains the patient's data, the diagnosis, the examinations carried out, the course of treatment, the doctor's recommendations, medicines, details of hospitalization and other important information. When translating Form 100, special attention is paid to the wording of diagnoses, Latin terms, test values, names of medicines, dosage, dates and the doctor's recommendations. Such a document is often used by a doctor to make a decision, so the text must not be superficial or carried over word for word in a way that loses the meaning.

Accuracy of medical terminology and Latin abbreviations

A Georgian medical document is rarely written in Georgian alone. In a single Form 100 you may find a Latin diagnosis, a Russian abbreviation, an English laboratory notation and a dose added by the doctor's hand at the same time. The translator has to decode each of these layers with the same precision, because a foreign doctor assesses the patient's condition precisely from the translation.

• Latin abbreviations - p.o. (by mouth), i.v. (into a vein), i.m. (into a muscle), s.c. (under the skin), b.i.d. (twice a day), t.i.d. (three times a day), p.r.n. (as needed). A mistake in a single letter changes the route or the frequency of administration;
• Doses and units - mg, mcg (µg), ml, IU. The difference between a milligram and a microgram is a factor of one thousand. We do not change or convert units: mmol/L stays mmol/L, so that the doctor sees the original value;
• Decimals - Georgian text writes "0,5 mg", English writes "0.5 mg". We adapt the decimal separator to the standard of the target language, the figure - never;
• ICD-10 codes - the code of the International Classification of Diseases (for example, I10 or E11.9) does not depend on language and is carried over unchanged. The verbal wording of the diagnosis is translated, in line with the official ICD-10 terminology;
• Russian abbreviations - ОАК (complete blood count), ЭКГ, УЗИ, в/в, в/м. Such notes are common in documents written by doctors of the old school and are spelled out in full;
• Medicines - the brand name stays unchanged. If the medicine is sold under a different name in the target country, where needed we add a translator's note with the international nonproprietary name of the active substance (INN).

Medical translation for treatment abroad

Translating medical documents for treatment abroad is especially important. The clinic must receive complete and clear information about the patient's condition, the diagnosis, the examinations carried out, operations, medicines and the history of treatment. In such cases the documents most often translated are Form 100, the discharge summary, the MRI/CT report, laboratory tests, the doctor's recommendation and the medical history.

Clients often send documents to clinics in Turkey, Germany, Israel, Italy, France, the USA or other countries. Some clinics need an English translation, others Turkish, German or another language. Medical translation for an embassy, a university and an insurer

Translation of medical documents may be needed not only for treatment, but also for official submission. For example, an embassy may require a medical certificate, a university a document confirming your health, an insurance company a doctor's report or test results, and an employer a certificate of health status.

In such cases the question often arises: is notarization needed? The answer depends on the receiving organization. If the document has to be submitted to an official body, notarization may be required. If the document is being prepared for a foreign state authority, sometimes you may also need to check the question of an apostille.

Does a medical certificate need notarization?

The same Form 100 is prepared in different forms for different recipients. The right format saves both time and money, while the wrong one often means you will have to prepare the document again.

• Notarized translation - the translator signs the translation in front of a notary, and the notary confirms that the translator's signature is genuine. This form is usually required by an embassy or a visa centre for a medical visa, a state body, a court, and some universities and insurance companies;
• Translation certified by the bureau - the translation carries the translator's signature and the bureau's seal. As a rule, this form is sufficient for a clinic abroad, for a remote consultation with a doctor, for obtaining a second opinion and for a preliminary assessment of treatment.

A foreign doctor needs accurate medical information, not state confirmation of the translator's signature. So if you are sending the document directly to a clinic, certification by the bureau spares you the extra step at the notary, and the translation is ready faster. If you later need the notarized form as well, the same translation will be notarized - you will not have to have it translated again.

• the recipient's requirements say "notarized" - you need notarization;
• they say "sworn" - in many countries this means a translator sworn in that country, so ask the recipient in advance whether a translation notarized in Georgia is accepted;
• they say "legalized" - you need an apostille or consular legalization; notarization does not replace it;
• they say "certified translation" or just "translation" - certification by the bureau is often enough;
• the document will be used in a foreign state procedure - check whether it needs an apostille.

Translation of pharmaceutical and clinical texts

Pharmaceutical translation differs from the translation of patient documents. Here what matters most is consistent terminology, the structure of the leaflet, accurate dosage, correct description of side effects, and clear rendering of contraindications and directions for use. Such texts may be used by a pharmaceutical company, a clinic, a distributor or a regulatory organization.

When translating clinical and research texts, attention is paid to the scientific style, terminology, accuracy of the data and preservation of the structure of the source text. If the text contains tables, study results, statistics or biochemical data, the translation must be as precise as possible and easy to work with.

A diagnosis, a psychiatric record, an HIV or genetic test result - these are the data whose disclosure can cost a person their job, insurance or reputation. The Law of Georgia "On Personal Data Protection" treats health data as a special category of data. So does the EU GDPR (Article 9). • Internal NDA - every translator and editor is bound by a confidentiality obligation;
• No artificial intelligence is used - the content of the document is not uploaded to AI systems or public machine translation services;
