Form 100 Translation for a Work Visa (Poland, Germany, USA)

When you go abroad to work, you will almost always be asked for Form 100 (form IV-100/a, the Georgian health certificate) - but not always by the embassy. Knowing this difference matters: it determines which language the certificate should be translated into, whether it needs notarization and when you should get it.

Below we look at the three most common destinations and at who asks for what in each of them. One note before you read on: consular requirements change, so always check the final list on the official page of the embassy or the visa centre.

Why a health certificate is requested

An employer needs to be sure that the candidate is able to do the specific job and does not have a contagious disease that would put other people at risk. That is why a Form 100 issued for work most often covers:

  • Tuberculosis - a chest X-ray or fluorography;
  • Infection markers - at the employer's request, for example hepatitis B and C;
  • General state of health - a general practitioner's conclusion on fitness for work;
  • Specialists' conclusions - often required in addition for work in the food industry, in healthcare or with children.

The main rule: the content of the certificate is determined by whoever asks for it. Before you go to the doctor, ask the employer or the agency for a written list, so that everything is entered in the certificate in one go. If the certificate does not match the list, you will have to repeat the tests, get the certificate again and have it translated again.

Poland

The consular list for a Polish national work visa (D) usually includes a work permit or the employer's declaration and medical insurance. A health certificate, as a rule, does not appear on it. Form 100 is mostly requested by:

  • the Polish employer or the recruitment agency - to select candidates, before the contract is signed;
  • specific sectors - food production, logistics, construction work.

Keep in mind that under the Polish Labour Code, before starting work an employee undergoes a preliminary medical examination (badania wstępne) in Poland, with an occupational medicine doctor. The Georgian Form 100 does not replace this examination, but at the selection stage it is often exactly what you need.

Language: Polish or English - as the employer requires. Certification: the agency's certification is usually enough. Some recruitment agencies ask for a notarized translation - ask in advance. About Polish translation: Polish translation.

Germany

For a German work visa, a medical certificate is generally not a mandatory document. A certificate confirming your health is required where the profession is regulated:

  • Nurses and carers - during recognition of the qualification, the competent authority usually asks for a doctor's certificate stating that your state of health is fit for the profession (ärztliche Bescheinigung). It often has to have been issued within the last few months;
  • Doctors - a similar certificate is part of the medical licence (Approbation) procedure;
  • At the employer's request - in some sectors, for example food production.
Language: German. About German translation: German translation.

Some German authorities ask for a translation done by a sworn translator in Germany (beeidigter Übersetzer). Others accept a notarized translation done in Georgia, with an apostille if needed. Before translating, check what your specific authority requires - it can be settled in a single email and saves you from having the document translated twice.

For nurses and doctors: the whole package together

Recognition of a medical profession in Germany or another European country does not rest on a single certificate. The health certificate is part of a larger package, which usually includes:

  • your diploma and transcript listing subjects and hours;
  • certificates confirming your work experience;
  • a professional licence or certificate, if you have one;
  • a criminal record certificate;
  • a health certificate;
  • a language certificate.

It is better to have all these documents translated together: your name, the name of your school and your job titles will then be written the same way in every translation, and the recognition authority will have no reason to ask about inconsistencies. The exact list must come from the authority that reviews your application.

How test results are written in the translation

In a certificate issued for work, the most important part is the results of the infection tests, and these are exactly what the employer checks first. In the translation they are carried over exactly as they appear in the original:

  • the name and abbreviation of the test unchanged - HBsAg, anti-HCV, RW and others;
  • the result word for word - "negative" stays "negative", not "normal" or "healthy";
  • the X-ray conclusion in full - "no focal or infiltrative changes are detected in the lungs" is translated in its entirety, not shortened to "no pathology";
  • the date of the test - the employer often checks how long ago the test was done.

The translator does not assess or generalize the result: the translation says exactly what the doctor wrote.

USA

For the USA there are two different situations:

  • Nonimmigrant work visas (H-1B, H-2B, J-1 and others) - a medical certificate is, as a rule, not on the embassy's standard list. Form 100 may be requested by the employer or the program sponsor;
  • Immigrant visas, including employment-based ones - the medical examination is carried out by a doctor designated by the embassy (panel physician), who sends the results directly to the consulate. The Georgian Form 100 does not replace this examination, although you may need documents on previous treatment or vaccinations.
Language: English. Certification: as the recipient requires - for an employer, the agency's certification is often enough. About the other documents for a work visa: what documents you need for a work visa.

How to prepare Form 100 for work

  1. Ask for the list - from the employer or the agency, in writing: which tests, which language, which certification;
  2. Get the certificate at the right time - many employers and authorities ask for a certificate issued within the last 1-3 months. A certificate obtained too early may be rejected;
  3. Ask for a printed certificate - handwriting makes translation harder and increases the risk of errors;
  4. Check your name - your name and date of birth on the certificate must match your passport. In the translation, your name is written exactly as it appears in Latin letters in your passport;
  5. Plan your other documents at the same time - for a work visa, a criminal record certificate, a diploma and a work experience letter are often translated too. With a single order, everything is ready within the same timeframe.

Fast notarized translation

A job offer often comes with a strict deadline: the employer is waiting and the visa centre appointment is booked. TARGMANEBI translates Form 100 into Polish, German, English and other languages the same or the next day. Where needed, notarization is completed within the same timeframe.

  • the translation is checked by an editor: the diagnosis, test results, dates and personal details are each compared separately with the original;
  • unclear handwriting is never filled in by guesswork - we mark such a passage and ask you to check it with your doctor;
  • only the translator and the editor see the document, and the content is not uploaded to AI systems.

Frequently asked questions

Will the embassy ask me for Form 100?

A health certificate, as a rule, is not on the standard consular list for Polish, German or US work visas. It is mostly requested by the employer, the agency or the qualification recognition authority. Always check the exact list on the official page of the embassy or the visa centre.

Do I need a notarized translation, or is the agency stamp enough?

It depends on the recipient. Employers and agencies often accept the agency's certification, while public authorities ask for a notarized translation. If you are not sure, start with the agency's certification - the same translation can be notarized later.

How long is Form 100 valid?

The validity period of Form 100 is set by the receiving party. Employers and authorities often ask for a certificate issued within the last 1-3 months, so get the certificate close to your departure date.

My certificate mentions a chronic condition. Can we leave it out of the translation?

No. A translation is a complete and accurate reflection of the original - the translator cannot remove or soften anything. If you have a question about the content of the certificate, discuss it with your doctor before the translation.

Will one translation work in two countries?

An English translation often works with several employers. If an authority in a specific country asks for the local language, for example Polish or German, you will need a separate translation.

Do I need another medical examination after arriving in Poland?

As a rule, yes. Under the Polish Labour Code, before starting work an employee undergoes a preliminary medical examination with an occupational medicine doctor. The Georgian Form 100 does not replace it - you need Form 100 at the selection stage.

A German authority asks for a translation by a sworn translator. What should I do?

Some German authorities accept only a translation by a sworn translator in Germany, others accept a notarized translation done in Georgia. Before translating, check in writing what your authority requires, so that you do not have the same document translated twice.

How long does the translation take?

The same or the next day, including notarization where needed.
